Safe Enough to Say It: Understanding Postpartum Harm OCD

CE Hours 1.5

About this live event

Disclosure, Assessment, and Response Through a Trauma-Focused Lens Unwanted intrusive thoughts of harm are common after childbirth, yet clients often hide them, and providers may respond in ways that deepen shame or feed compulsions. This introductory training helps therapists and other perinatal and reproductive providers recognize obsessions and compulsions in context, including reproductive history and cultural meaning. Participants will compare harm-related obsessional thoughts with postpartum psychosis and other co-occurring presentations, and will review assessment considerations within their professional scope. The session teaches communication that supports disclosure without reinforcing reassurance seeking, and it outlines when to consult, refer or arrange urgent evaluation. Content draws on peer-reviewed perinatal OCD and postpartum psychosis literature, the presenter's clinical perspective, and fictional clinical scenarios.

Learning Objectives

  • Distinguish an isolated intrusive thought from a clinically significant obsession-compulsion pattern by naming at least three features, such as distress, persistence, time consumed, impairment, avoidance and compulsions, including hidden mental rituals.
  • Describe assessment considerations (intent, planning, behavior, reality testing, functioning and ability to maintain safety) for differentiating perinatal OCD from postpartum psychosis and from co-occurring depression, anxiety and trauma-related symptoms.
  • Explain how reproductive history and cultural or religious context can shape the content and meaning of perinatal symptoms without pathologizing cultural practices.
  • Apply at least two communication strategies that support disclosure while avoiding reinforcement of compulsive reassurance seeking.
  • Identify indications for consultation, specialist referral or urgent evaluation within their professional scope.
